Spammers do not sit there filling in new registrations for forums, even paying someone to do it in third world countries is an inefficient option compared to writing (or purchasing) software that spiders the web for new forums using standard code or providers (eg Yahoo, phpBB) and automatically posts an advert. If it's necessary to create an account to post, it does that automatically too, creating a random username to make a single post (the next time it posts will be under a fresh randomly created username - it's easier to create new ones than store old ones, which will likely be immediately banned anyway).
The best way to beat the bots is to make sure your phpBB installation is kept patched and up to date (security holes exploited by hackers can then be closed when they're discovered and fixed) and then to take your pick of the permission options available. Not allowing guest postings will foil the simplest spambots, the two options to foil automated sign-ups are e-mail verification (which I dislike as it ironically tends to lead to trouble with e-mail spam filters), and visual confirmation, where a mad image of a code is shown, all skewed and curvy (to frustrate image scanners) which you have to enter, proving that the signup is the work of a human being. Though it can sometimes the image can be a little too mad to read even for a human (though if you get it wrong it simply shows you another) this is my favourite option and the one we use on the Wamdrogony board, now that phpBB have got it working properly.
But everyone, please stop posting threads of flame responses to every spam message that appears. The poster is a bot, it cannot read, has no mind and can only do as it is programmed. Expending the energy to send it insults just makes us look like the old guy in the park threatening the rainclouds.

I always thought there was just one guy locked away in a dark room going around all the sites one by one
It might be a plan to update the version of the forum so that it has those "mad images of a code" though. Not that I've seen much spam to be honest as it looks like Hayleys "clean up" skills must be well honed from her wamming!
